Note: Reports for other programs or those older than five years are available upon request at our office.
Understanding Inspection Reports
A “Snapshot” in Time
An inspection report represents conditions only at the time of the inspector’s visit. It is a snapshot of that specific day and may not represent the overall, long-term operations of a facility.
Look at the History
To get a more accurate picture of a facility’s commitment to safety and compliance, we recommend reviewing their inspection results over a period of time rather than looking at a single report.

How to Find a Specific Report
Follow these steps to locate records in the Accela Citizen Access Portal:
- Accessing Records
- Open the Portal and select the Environmental Health tab.
- You do not need to log in to view public reports.
- Search Criteria
- Select Search Applications/Records.
- Search by Facility Name, Address, or Record Number.
- Viewing the PDF
- Click on the blue Record Number for the facility.
- Navigate to Record Info > Attachments.
- Click on the Inspection Report PDF to open or download the record.
Body Art, Campground, Food Establishment, Private Sewage System and Recreational Water inspections After September 2025 are available on the website. Inspections from the previous five years and for other programs are available at the Warren County Health District upon request.
